Liability Waiver Form

Please fill out the following form.

I, the undersigned parent of the athlete registering in an athletic development program provided by Bilbrey Training Studio, hereby acknowledge and agree to the following terms and conditions of this Waiver of Liability:

1. Nature of Services: I understand that I am registering my child into an exercise/athletic development program that requires physical activities and fitness equipment provided by Bilbrey Training Studio for the intentions of physical and cardiovascular improvement.

2. Assumption of Risk: I acknowledge that the program I am registering my child in involves certain risks and hazards. I assume all risks associated with the program I am registering my child to participate in, including but not limited to accidents, injuries, property damage, and any other risks inherent in the nature of the exercise/activities performed in this program.

3. Release and Indemnification: In consideration of being a participant in a program provided by Bilbrey Training Studio, held in the location owned by the Applause Group, LLC., I hereby release, discharge, and hold harmless Bilbrey Training Studio,the Applause Group, LLC., it's officers, directors, employees, agents, and affiliates from any and all claims, demands, actions, causes of action, liabilities, costs, expenses, or damages of any kind that may arise out of or in connection with my child's participation in the program he/she is registered in.
